Please bring the following items with you to register your child for school: • Legal Proof of Age • Certificate of Immunization (Florida HRS 680) • Filled out registration forms • Evidence of Medical Examination (well check-up) within the last 12 months of beginning school • Two (2) Proofs of Residency • Documentation of Grade Placement • You will need to obtain the emergency contact card and the Student Code of Conduct from the school Please bring the following items with you when you withdrawal your child from school: • Filled out withdrawal form • All textbooks that were given to your child to take home • All library books that were checked out by your child |
